You will be an excellent communicator, have the ability to work autonomously whilst being part of a team with an integrated approach to care. You will have well-developed clinical skills and be able to lead within the context of a Community Nurse role. You will provide high quality clinical care, supervision of staff and case management for patients in the community.
The team strives to provide patient centred care, and actively encourage prevention and self-management, therefore the applicant will be required to work closely with the multi-disciplinary team, working collaboratively, and strengthening relationships with our GPs.
Community Nursing is a highly specialised area of practice recognised by the Queens Nurses Institute and NMC as a specialist role. As a District Nurse/Senior Community Nurse, you will work within an integrated hub model of District Nurses, Specialist Nurses, Community Staff Nurses, Therapists, Trainee/Registered Nursing Associates, Associate Practitioners, Health Care Assistants and phlebotomists to meet the needs of the complex patient in the community and avoid admissions to the acute sector as far as practicable.
