PRACTICE NURSE

(WITH CHRONIC DISEASE MANAGEMENT)

PART TIME POSITION- 24 HOURS PER WEEK 

We are looking to recruit a dynamic, forward thinking and skilled practice nurse.  Applicants should have experience of managing chronic diseases (diabetes, chronic heart disease, asthma and COPD).  We are a friendly practice, with a CQC rating of good, with outstanding for caring. 

The clinical team consists of a Lead GP, two Salaried GP’s, two ANP’s, two practice nurses and a HCSW.  You will be required to work over both sites ie Saxilby and Torksey, we are a dispensing practice. 

Flexibility, good team work, enthusiasm and working autonomously are important for this role, although you should have a good send of humour, an empathetic and caring personality are also necessary.

We are a SystmOne practice, using Ardens Templates, knowledge of these would be desirable but training would be given.

Generous benefits with regard to uniform, NHS pension scheme, annual leave.  Rate of pay is dependent on experience.
