An exciting opportunity for a Junior Sister has arisen at Worcestershire Acute Hospitals NHS Trust. As part Worcestershire’s Integrated Care of Older People (ICOPE) Pathway we provide integrated care for people living with frailty by working together. We ensure that frailty is recognised early in the acute hospital setting and work with partners in all care settings and that people living with frailty have:
The role of professionals who specialise in care of older people is crucial in identifying and implementing best practice so our ICOPE pathway grows into an exemplar of person-centred care.
Worcestershire Acute Hospitals Trust is working with partners, including neighbourhood teams, to grow its acute service for older people living with frailty that embeds Comprehensive Geriatric Assessment. A talented, enthusiastic and motivated multi-disciplinary team of Practitioners, Nurses, Physiotherapists, Occupational Therapists, Pharmacists and Social Workers work alongside Consultant Geriatricians to deliver the service.
Working closely with both the Trust’s Emergency Departments in Redditch and Worcester, the service also takes direct referrals from West Midlands Ambulance Service, as well as from GPs and other services within the Trust. Referrers can obtain telephone advice and guidance via Consultant Connect. The Trust has invested to extend the operation of the current service to 7 days to increase its capacity.
You will be a strong team player, keen to work in a multi-disciplinary team, using advanced assessment skills and knowledge of complex patient needs to facilitate same day discharge. You should be passionate about developing outstanding integrated services for older people.
