Hours:Monday – Sunday 08:00 – 20:00, 08:00 – 16:00, 12:00 - 20:00

Job Summary:

  • The post holder will be a key member of our multidisciplinary team to provide an urgent community response to patients who are experiencing a health crisis with the aim to support them in their own homes (including care homes) rather than being admitted to hospital.
  • Provide a 2-hour urgent response for patients who are entering a health crisis.
  • Provide advanced levels of assessment to investigate, diagnose and treat patients in their own environment and prevent unnecessary hospital admissions.
  • Work in collaboration with other members of the MDT
  • Work flexibly in response to service need across multiple sites.

The “must haves” for you to be considered for this role:

  • You must be a Registered General Nurse.
  • You must be a Non-Medical Prescriber.
  • Have an advanced history taking and physical assessment qualification.
  • Current registration with the NMC.
  • Enthusiastic and committed to developing skills and knowledge.
  • A team player with the ability to work collaboratively with other disciplines.
  • Car driver with ability and readiness to travel to multiple sites.

System Knowledge:

  • Rio.
  • Microsoft Outlook.
  • ICEtiple sites.
  • Adastra (Training given).

Education/Qualifications/Training:

  • A recorded/registered qualification in one of the following at graduate level in nursing, social work, occupational therapy.

To apply for a post, you must:

  • Demonstrates an understanding of anxiety and depression in Primary Care settings.
  • Demonstrates a knowledge of the issues surrounding work and the impact it can have on mental health / benefits & employment systems.
  • Knowledge of medication used in anxiety and depression and other common mental health problems.
  • Demonstrates an understanding for the need to use evidence based psychological therapies and how it relates to this post.
  • Experience of working in mental health services.
  • Ability to meet agreed/specified service targets.
  • Ability to manage own caseload and time.
  • Demonstrates high standards in written communication.

Please note:

From 1 April 2022 it will become a legal requirement for health and social care staff who have direct, face-to-face contact with patients/service users to provide evidence of a complete course of an authorised COVID-19 vaccine unless they are exempt.

This requirement also extends to non-clinical staff not directly involved in patient/service user care, but who may have direct, face-to-face contact with patients/service users in the course of their work.

Applicants who fall within the scope of the legal requirements will be asked to evidence their vaccination status, or valid exemption, as part of the recruitment process.
