Community Staff Nurse - Band 5 - COVID-19 in Stoke-on-Trent

Job description

We are one of the largest integrated Health and Social Care providers, with an annual turnover of close to £400m, serving a population of 1.5m over a core geography of 2,400 square miles. Our portfolio of services includes mental health, learning disability and adult social care services across Staffordshire, Stoke-on-Trent and Shropshire. We have specialist mother and baby, eating disorder and medium secure forensic mental health services and contracts across the country for IAPT, addiction and some health services in prisons as well as hosting a network for inpatient mental health care for serving military personnel.

This is an exciting opportunity for level 1 (adult) registered nurse to become part of rapid response a 24 hours, 7 days a week team, where we offer nursing interventions to patients living in their own homes. We work in an integrated way with health and care providers to ensure that care is joined up and that whenever we can, we strive to care for patients at home, helping them to maintain independence and ensuring that high quality care is delivered in an efficient and holistic manner.

  • Demonstrate your passion and ability to assess, plan, provide and evaluate evidence based nursing care for patients experiencing a wide range of conditions.
  • Support our patients and work with people in an asset-based way that really enables them to live their optimum life, being self-caring and independent with their health needs where possible.
  • Work collaboratively, efficiently and creatively with multi professional colleagues and partners with a focus on helping patients to avoid hospital admissions and where they do need a hospital stay facilitating timely and well supported discharge back home.
