About The Role Closing date: 12th February Location: Community and home based in the Cheshire area. About the role Are you passionate about making a positive difference in the lives of people affected by dementia? Do you want a career where every day is different and full of reward? Yes, well, we would love to hear from you! The role is delivered in the community, based from home. You will be required to carry out home visits, deliver a range of groups and attend meetings across the area. The Cheshire East Dementia Support Service provides a highly responsive and individualised information, signposting and referral service to people living with a dementia diagnosis, their carers and family members. We also deliver a range of community-based groups including our successful Singing for the Brain groups across the Cheshire East area and we are looking to develop new groups that will add to and enhance our support offer.

Support and role manage the group of trained volunteers that help us to deliver the groups. Provide information and guidance to help maintain peoples independence, improve their sense of well-being, and put them more in control of their lives and those that care for them. Be part of a highly motivated and supportive team, Build relationships with a range of local contacts, networking with health and care professionals, marketing the Cheshire East Dementia Support Service and the groups we host. Develop networks and pathways with partner organisations who work in the field of dementia, ensuring seamless and consistent support and empowering people affected by dementia to make informed choices. Provide support face-to-face, by telephone and online. About you We are looking for a highly motivated individual, with an understanding of dementia and the needs of people affected by dementia. You will;

Be empathetic and non-judgmental in your approach with a commitment to equal opportunities. Have excellent communication skills to meet the diverse needs of our community and represent their needs to statutory and other voluntary agencies. Have a sound knowledge of IT systems to record data, write reports and communicate by email. Have experience of running and planning groups and working with volunteers Understand client confidentiality and how this is applied when representing client needs. Preferably had some experience of working with a wide variety of relevant agencies. Possess NVQ level 3 (or equivalent) or have demonstrable experience. Have the ability and means able to travel independently around Cheshire West & Chester
