CARE COORDINATOR (HOME CARE)

Starting Salary:- £26K P.A. - £28K P.A

Bracknell, Berkshire

We are working with a highly regarded Home Care provider based in Bracknell, Berkshire, who is looking to recruit a passionate and motivated individual to join their growing team.

This is a fantastic opportunity to step into a rewarding Care Coordinator role, where you will play a key part in delivering high-quality care to clients within the local community. You will make a real difference to the lives of predominantly elderly individuals, supporting them to remain safe, independent, and comfortable in their own homes.

Reporting to:- Registered Care Manager


Duties will include:-


  • Care Coordination/Rostering/Scheduling


  • Prioritize well being off both Care Staff and Service Users when scheduling


  • Care Plans


  • Risk Assessments


  • Deliver a Safe & Compliant Home Care Service within CQC Guidelines, provide the utmost Customer Service to Services Users and their Families at all times


  • Care Worker Spot Checks


  • Administration


  • Safeguarding of both Care Workers & Clients (Service Users)


  • Liaise with Key Social Care Parties:- Social Workers, GP Practices, NHS Discharge Teams, CQC


  • Deliver Personal Care to Clients (Service Users) in the community when

needed in order to comply with CQC regulations


  • Be part of a rota'd Team On Call Service



Working Hours:

Office Based:

Monday - Friday 09:00hrs - 17:00hrs

or

Monday - Friday 08:00hrs - 16:00hrs

or

Monday - Friday -07:00hrs - 15:00hrs


On call rota service 1 weekend in 6 weeks



Desired experience:

Care Coordination/Rostering/Scheduling experience

Strong relationship building skills and empathetic nature

Home Care Experience (Carer)

Proficiency in following CQC regulations, Safeguarding Measures
