We have exciting opportunities for highly motivated, enthusiastic nurse to work with the Rapid Response Team based at London Road Community Hospital.

Our team focus on visiting patients in their own homes, with an urgent nursing need or on the discharge to assess pathway. It is essential that you have a keen interest and some experience in community nursing and /or complex assessment .

The successful candidate will also have extensive clinical skills and experience of caseload management.

If you can demonstrate good communication skills, have the ability to plan and organise competent, evidence based clinical care delivery we want to hear from you.
