Job description

Durham City East Community Mental Health Team are looking to recruit a Band 3 Community Support Worker – Physical Health. The position will be based at Chester-Le Street Health Centre with the new and developing Community Mental Health Team.
The role of Physical Health Support Worker will include supporting patients within the team to improve quality of life and promote recovery in relation to mental health, physical health, and well-being.
The successful candidate will be confident in the monitoring of general physical health/lifestyle factors and the monitoring of the medications Clozapine and Lithium. The role requires skills and knowledge in venepuncture, ECG, blood pressure monitoring and HDAT monitoring. Training and support will be provided as needed to develop skills relevant to the role.
The successful candidate must have completed the Diploma in Health and Social Care Level 3 or equivalent. Candidates must also have Literacy and Numeracy level 2 or equivalent and must complete the Trust Healthcare Assistant competencies within their probationary period (first 6 months in role).
Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010
